The patient is a 28-year-old usually healthy man who developed acute onset of diaphoresis, nausea, and severe chest discomfort. He called 911 and was brought to the emergency room. His past medical history and family history were unremarkable, his physical exam was normal, and he took no medications and had no history of tobacco or illicit drug use, with occasional alcohol use. An electrocardiogram (ECG) showed sinus tachycardia at 100–ST elevation in the lateral leads. The patient was then taken emergently to the cardiac cath lab.

A 55-year-old man presented with complaints of angina with maximal exertion. The patient had been referred by his family practitioner after his wife insisted he get a check-up. The referring physician performed a stress test that was not diagnostic.
