This statement of privacy ("Privacy Policy") describes our commitment to your privacy and our information gathering and dissemination practices for www.endovascular.org ("Website"), a website operated by Endovascular Forum ("EVF"). We urge you to read the following information so that you will understand both our commitment to you and your privacy, and how you participate in that commitment.


Voluntarily Provided Information

When you become a registered user ("Member") of the Website, you voluntarily provide personal information to EVF. EVF may also collect other information that Members voluntarily provide to us through responses to surveys, postings on message boards, or through online feedback mechanisms. We will continue to use Members information as described in this Privacy Policy unless and until a Member notifies us that they do not want their information used by EVF.

Members voluntarily provide information such as their name, specialty, institution, mailing address, zip code, and email address. We require Members to submit a valid email address so that we may contact them about changes to this Privacy Policy and other important matters.

A Member must create and use a username and password to verify the users identity when logging into the Website. Members may modify their password and other personal information at anytime by logging into the Website with their unique username and password and selecting "My Account".

Release and Use of Your Information

EVF is the sole owner of the information voluntarily provided to us by Members. We will not share with any third party any information (such as name, email address, or mailing address) that can identify a Member personally, or allow a third party to contact a Member, except under the special circumstances described in the next paragraph.

EVF may share personally identifying information with a Members direct, explicit permission. EVF may share personally identifying information in other special cases when we have good reason to believe that sharing this information is necessary to identify or bring legal action against someone who may not be following the EVF terms and conditions or may be causing injury to (either intentionally or unintentionally) other Members, EVF itself, or anyone else that could be harmed by such activities. Lastly, EVF must cooperate and share personally identifying information if required by applicable law.

EVF may do internal research using the information provided to us by Members when registering, our system activity files or any other information that our Members voluntarily provide to EVF. EVF will analyze only information about users that is stripped of any personally identifying information (such as name, email address, or postal address) and then summarized so that our employees do not access users' personally identifying information. EVF may share some of this summarized information, containing no personally identifying information, with business partners. If a Member has agreed to allow EVF to communicate with them, EVF may use the Members information to communicate with them as a user of our services. Members may choose to remove themselves from future mailings at any time (please see the opt-out section below).

Our Opt-out Policy (How to Ask Us Not to Use Your Information)

At registration, EVF provides Members the opportunity to opt out of receiving any communication from us. We also give all of our Members the option to remove any and all of their information from our databases and to not receive future communications by sending an email to memberservices@endovascular.org.

Security

EVF takes all reasonable steps to protect the personal information of Members of this Website. The servers on which we store personally identifiable information are owned by EVF and kept in a secure location.

If others have access to a Members computer, Members should log out and close their browser when finished accessing the Website. This action prevents other people from accessing a Member's information.

Member Responsibility for Protecting Information

If you are a Member of this Website, it is solely your responsibility to control any and all access to your username and password, to log out when you leave the Website, and to immediately inform EVF of any reason to disable your username and/or password.

Public Forums

EVF provides Members of the Website with message boards and other public forums. Please remember that any information disclosed on these public forums becomes public information and Members should exercise caution when deciding to disclose any personal information. In addition, Members should exercise caution and be aware of professional ethical guidelines and laws that restrict the disclosure of certain personal and health information about patients. Any information (including personal and medical information) that Members reveal in the Website's public forums is not subject to this privacy statement and will be seen by third parties not related to EVF.

Our Use of IP Addresses

EVF collects and stores Internet Protocol (IP) addresses. We collect IP addresses to maintain and analyze our system. An IP address is a number automatically assigned to your computer whenever you access the Internet. All computers are identified on the Internet using IP addresses. IP addresses allow computers and servers to recognize and communicate with each other. EVF's use and analysis of IP addresses does not personally identify Website Members. However, EVF may attempt to use IP addresses to help locate users who refuse to follow this Website's Terms of Use and to identify users who threaten our service, site, customers, or others.

Our Use of Cookies

We may also collect information about Members and their use of the Website through the use of "Cookies". Cookies are small computer files that are transferred to a computer's hard drive from websites. Cookies allow websites to display content on web pages specifically tailored to each user. If you do not want information to be collected through Cookies, there is a simple procedure in most browser software programs that allows you to reject or accept Cookies. However, since our Website is personalized for each Member, our Website requires the use of Cookies for you to use many of the Website features. If you link out of our Website to Third party websites, they may also use Cookies. We do not control third parties use of Cookies or how third parties use the information they gather.

External Links to Third Party Websites

EVF may provide links to third-party external websites as a convenience to Members. It is important to know that this Privacy Policy does not cover any of these external websites. These third party external sites have their own privacy policies. EVF has no control over the privacy policies or content of any third party external websites. Please be sure to check the privacy policies of each site you access through our Website, as they may differ.

Revisions to Privacy Policy

If a Member has supplied EVF with a valid email address, EVF will send notification to the email address supplied by the Member if there is a significant change in this Privacy Policy. We will also post a changed Privacy Policy on our Website thirty days prior to using a new Privacy Policy. For any Member who chooses not to agree to this or any new Privacy Policy, please refer to our opt-out policy above.
